Physical Therapist

About the Position:
We are seeking a skilled and dedicated Physical Therapist to join our team on a casual basis for weekend and weekday coverage. This role involves providing comprehensive therapy services to patients of all ages, including evaluation and assessment, individualized interventions, education, treatment planning, and consultation. The goal is to enhance patients' abilities to achieve their highest possible level of function. You will work collaboratively as part of a care team committed to achieving optimal clinical outcomes, under the direction of the referring physician.
Essential Duties:
  • I nitial Evaluations/Assessments: Conduct initial evaluations and re-assessments based on medical provider orders and facility policies. Gather and interpret relevant data to provide appropriate care.
  • Plan of Care: Develop and update individualized care plans based on patient diagnosis, physical and cognitive status, functional goals, and discharge plans.
  • Treatment Delivery: Provide treatments tailored to each patient's physical, cultural, and psychosocial needs as outlined in the plan of care.
  • Patient/Family Education: Plan and deliver education to patients, families, and caregivers. Identify learning needs, communicate clearly, and document responses to the education provided.
  • Documentation and Billing: Complete daily documentation and billing in compliance with current standards and policies.
  • Compliance: Adhere to hospital Joint Commission, Department of Health, and Medicare standards.
  • Discharge Planning: Collaborate with the interdisciplinary team to plan for patient discharge. Assess patient needs for both short-term and long-term outcomes and ensure proper preparation for discharge.
What we are looking for:
Education and Experience:
  • Bachelors, Masters, or Clinical Doctorate in Physical Therapy from an accredited institution.
  • Valid Physical Therapy license to practice in the State of Minnesota.
  • Current BLS/CPR certification.
License/Certificates:
  • Physical Therapy license to practice in the State of Minnesota.
