Job Description & Requirements Specialty: Clinic Discipline: RN Start Date: ASAP Duration: Ongoing Shift: 8 hours, days Employment Type: Staff Overview: Community Health Care Center is seeking a full time RN Services Manager to support the LGBTQ facility in Asbury Park, NJ. Responsible for providing coordination of clinical care of Ryan White and LGBTQ Center for Health & Wellness patients including lab work, vaccines, and referral to outpatient specialty care as well as collaborating with the RW/LGBTQ team to provide appropriate client referrals. Responsibilities: The core responsibilities for the RN Services Manager wil be: Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers Primary clinical point of contact for all patients. Provides initial nursing assessment of all new patients’ needs, verification of HIV status when necessary. Coordinates clinical care for all patients, liaison between patient, care provider and team. Oversees case management for medically complex patients, advocating for clients with insurers, subspecialty clinicians, substance use, mental health and hospitals and rehabilitation facilities. Educates patients at every visit on HIV/STDs, safe sex, prevention and treatment Educates patients and families about pertinent medical issues, including assessment for referrals for PEP and PREP services. Maintains familiarity with community resources to meet client needs. Assists team in identifying patient care improvement needs and participates in CQI activities Adheres to infection control and safety policies and guidelines of CHC regulatory agencies Provides timely and complete documentation in all relevant data bases including CareWare. Adheres to the organization’s policy in regards to absenteeism and appearance Supervises RW/LGBTQ clinical team in conjunction with the organization’s managerial/leadership policies and procedures Performs nurse visits as ordered by providers for injections, vaccinations, labs, or phlebotomy. Qualifications: Job Competencies & Minimum Qualifications : Licensure as RN in NJ and Nursing Degree from NLN accredited program Excellent interpersonal skills Computer skills HIV/AIDS work related experience & knowledge of LGBTQ community cultural and lifestyle issues preferred Spanish Bilingual preferred or proficiency in Current professional CPR/BLS certification Working Conditions/Physical Demand : Working indoors and outdoors with equipment including Thermometer, sphygmomanometer, stethoscope, otoscope, opthalmoscope, EKG machine, venipuncture equipment, spirometry treatment, culture mediums, telephone, fax, photocopier, computer, printer and automobile.
