RN/LPN - Private Home Care

Our home health care agency is rapidly expanding, and we are hiring a Home Health Nurse effective immediately. We work with patients of all ages and backgrounds who require daily care in their homes. Most of our patients have suffered severe injuries, so we hire the most compassionate and thoughtful talent in the field. Responsibilities:

- Assess patients’ condition during every visit and chart your observations.

- Perform evaluation tasks, including vital signs and medication review.

- Administer medication as prescribed by the patient’s Physician.

- Maintain and update client clinical records as required to assure compliance with company policies, state/federal regulations and professional standards of practice.

- Provide case coordination by communicating with team members both verbally and in writing to ensure quality of care and client satisfaction.

- Performs other related duties as required.

Skills:

- Ability to effectively communicate with patients, families and hospital personnel is required.

- Strong interpersonal and customer service skills required in order to interact effectively with individuals from various cultural and socio-economic backgrounds.

- Excellent communication and time-management skills

- Must have reliable transportation and valid driver's license.

- Must be able to lift, carry, transport and move clients, if needed.

- Home Care experience required.

- RN/LPN license required.
