This position will be responsible for providing leadership to and mentoring all assigned CNA staff, while performing all CNA duties. Essential Job Functions: <ul><li style="text-align:justify;">Practice the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center and adhere to the MOSES program to include Mission, Ownership, Success, Excellence and Service</li> <li>Maintain confidentiality of all work-related information according to HIPAA regulations.</li> <li>Responsible for mentoring, training, counseling, assisting with performance evaluations and providing recommendations to the CNA Supervisor for disciplinary direction and performance improvement as needed. </li> <li>Serve as a leader within the nursing department by leading by example and exhibiting professionalism while performing duties</li> <li>Work alongside all CNA team members to monitor, mentor and enhance their skills.</li> <li>Mentors proper documentation to CNA team members</li> <li>Proficient in and able to perform all CNA duties, including but not limited to: <ul style="list-style-type:circle;"> <li>Ensures that CNA team members are engaging Neighbors in meaningful activities each day.</li> <li>Provides for privacy while giving personal care.</li> <li>Provides physical comfort by assisting Neighbors with activities of daily living. </li> <li>Gives mental, emotional, and physical support in accordance with care plans. </li> <li>Always treats neighbors and team members with dignity and respect.</li> <li>Demonstrates patience, compassion, and respect towards team members, neighbors, and encourages neighbors to be mentally active and as independent as possible. </li> <li>Provides meals, snacks and fluids and assists neighbors to eat when needed.</li> <li>Documents all care personally given as well as periodically or as needed.</li> <li>Provides preventative skin care/pressure ulcer prevention measures; i.e., turning, repositioning, shifting weight, bridging, special cushions and monitoring.</li> <li>Performs basic vital sign skills: i.e., Temperature, Pulse, Respirations, and Blood Pressure.</li> <li>Assists with ambulation or mobility needs using the proper lift and/or gait belt according to established lift policy. </li> <li>Oversees and enforces routine cleaning of equipment such as over bed tables, commodes, bed pans, wheelchairs, etc. Keeps work areas neat and clean. </li> <li>Performs rounds with and reports to on-coming shift.</li> <li>Maintains awareness of changes in Neighbors’ condition and appropriately reports abnormal changes to the charge nurse.</li> </ul> </li> <li>In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon Eben Ezer needs or requirements.</li> </ul> Qualifications: <ul><li>To perform this job successfully, an individual must be able to perform each essential job function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Will have training in safe food handling. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. </li> <li>Minimum of two years of experience working in a skilled nursing community.</li> <li>Experience in training and supervising others in a SNF environment preferred.</li> <li style="text-align:justify;">Must be able to read, write, and follow verbal and written instruction. Accuracy and clarity are a must.</li> <li style="text-align:justify;">Must have good interpersonal skills. </li> </ul> Education and Experience: <ul><li style="text-align:justify;">Graduate of an approved Nurse Aid program and hold a current Colorado CNA license. </li> <li style="text-align:justify;">Understands the Eden Alternative Principles and is able to lead others in implementing person-directed care.</li> </ul> Non-Essential Job Functions: <ul><li>May participate in Eben Ezer committees.</li> <li>Will attend community meetings and department meetings as instructed.</li> <li>Will assist the CNA Supervisor with education planning and demonstration for the CNA team</li> </ul> Other Skills and Abilities: <ul><li>Must be able to work independently as well as within a team</li> <li>Ability to hold team members accountable and maintain confidentiality</li> <li>Must be able to communicate effectively both orally and in writing. </li> </ul> <ul><li>Ability to manage time and resources effectively.</li> <li>Maintain punctuality when reporting to work and leaving work.</li> <li>Must demonstrate the willingness and ability to learn and perform all duties required by the Nursing Department.</li> <li>Ability to work and converse in a polite manner with neighbors, their families, co-workers, and others associated with the position on a daily basis.</li> <li>Must be able to follow safety measures to protect neighbors, self, and co-workers. </li> </ul> Material and Equipment used but not limited to: <ul><li>Equipment associated with patient care such as lifts, tubs, wheelchairs, Geri chairs, gait belts, specialty beds, blood pressure cuff, etc. </li> </ul> Working Environment/Physical Activities: <ul><li style="text-align:justify;">Must be able to push/pull up to 250 pounds regularly using a wheelchair.
