Job Description & Requirements Specialty: Clinic Discipline: RN Duration: Ongoing Employment Type: Staff Lead for CCP Value Reporting– Serves as primary contact, follows up on outstanding items– leads and supports development/evolution value based reporting with payers, communicates with department leaders regarding status and follow-up, Produces and provides reports for leadership LeadsPayer Platform Relationship– Serves as contact, liaison with Practice Assist, Data Link and Forvis for database entry and storage issues, runs and distributes reports and as requested by department leaders, , assesses patient needs and directs issues/concerns to appropriate care team members for outreach provides patient and caregiver follow-up data, Maintains historical database and tracks trends/patterns. Assembles data and produces reports for leadership. Leads Quality Reporting for Physician Compensation- provides oversight and needed reporting to support quality component of physician compensation for clinic system Effectively communicates with providers, patients, and multi-disciplinary care team members via reporting, care gap closure and educational presentations Projects a caring, friendly, knowledgeable and friendly image to providers and care team members.
