This position will work in Hyde Park, Roslindale, Braintree and Quincy. Job Summary Provide for the social, economic and psychological needs of patients and their families utilizing social work skills including direct service, consultation with the interdisciplinary team, and referral to community resources. Key Activities • Patient needs are met by planning and carrying out the treatment plan in accordance with the patient’s developmental stage, psychosocial concerns, cultural background, illness, and living environment • Patient population is managed in accordance with medical social work assessment and treatment techniques. • Teaching is provided from time of admission and evaluated on an ongoing basis to meet the needs of patients and families learning style and diagnosis. • Plan of treatment is regularly reviewed and modified to meet discharge goals. • Relationships with community agencies are developed and strengthened in order to best meet needs of patients and families. • Documentation is completed to support agency and regulatory guidelines, including third party payor requirements • Physicians’ orders are current and consistent with the plan of care and are documented in the patient’s medical record. • Social work practice is updated through participation in appropriate in-services, conferences, and workshops Qualifications and Experience • Master’s Degree from a School of Social Work accredited by the Council on Social Work Education • Minimum two years social work experience in a health care setting • Licensed by the Board of Registration in Social Work at the Licensed Independent Clinical Social Worker (LICSW) level or Licensed Clinical Social Worker, pursuant to G.L. Chapter 13, Section 80.
