RN Care Team Coordinator (Full-Time)

Job Summary:
An RN CTC works within the interdisciplinary team to create comprehensive individualized plan of cares for assigned patients.
Schedule: Monday - Friday 8:00a-4:30p (No Weekends or Holidays)
Location: Wooster OH
Qualifications:
· Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred
· Active Registered Nurse license in the State of Ohio without any board actions.
· Two years acute care nursing experience preferred
· Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
(Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
· CPR Certified
· Computer skills sufficient to properly document services and care.
· Ability to drive during daytime, nighttime or inclement weather.
· Valid Driver's License with Safe Driving Record
· State Minimum Automobile Insurance Coverage
Key Responsibilities
· Demonstrates working knowledge of all Conditions of Participation and Joint Commission Standards to ensure that Ohio’s Hospice’s medical records meet regulatory compliance standards.
· Develops and implements an individualized plan of care including creating measurable patient-centered goals in collaboration with the interdisciplinary team members, that consistently is updated and reviewed to reflect assessment findings to ensure quality patient care.
· Collaborates and participates as member of hospice team in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings, to ensure coordination of services.
· Participate in Continuous Quality Improvement activities to support the quality of Ohio’s Hospice services.
· Precept new staff to support professional relationships with newly hired team members as required.
