Position: RN, Care Coordinator

Department : Med-Surg/ED

Hours: 40-hour work week with requirement of working every third weekend (7am to 7pm on Saturday and Sunday) on Med/Surg/ED.

Position Summary:

Responsible for the coordination of utilization review, discharge planning, skilled care management, and the SHIIP program. Matches patient care needs to available resources, ensures the patient stays at the appropriate level of care and adjusts if necessary. Accountable for the coordination of care of inpatient, outpatient, and skilled services to reach ideal clinical and patient care outcomes while incorporating all available resources as needed. Works collaboratively with providers, staff, and other health care professionals to provide comprehensive care management and care coordination across the health care continuum. Works to ensure members can navigate through the health care continuum by improving the coordination of care and patient/family experience. Works with patients and their families to address concerns or special needs that may arise during their course of treatment within the hospital setting. 40-hour work week with requirement of working every third weekend (7am to 7pm on Saturday and Sunday) on Med/Surg/ED.

Qualifications:

  • Registered Nurse licensed in the State of Iowa. Bachelor’s degree in nursing preferred but not required.
  • Experience working with health insurance, case management, transitions of care, information technology.
  • Highly developed problem-solving skills.
  • Ability to adapt to flexible work schedule and frequent interruptions.
  • Ability to prioritize to meet deadlines on daily work and special projects.
  • Must be able to work independently with minimal supervision.
  • Effective oral and written communication skills, excellent interpersonal skills.
  • Must exhibit a high degree of responsibility for confidential matters.
