Reintegration Peer Support Full Time GENERAL RESPONSIBILITIES & ESSENTIAL JOB FUNCTIONS: <ul><li>Provide Peer Support services to persons in jail who are willing to engage in mental health and/or substance use recovery services.</li> <li>Contact with new referral is made within 48 hours of incarceration</li> <li>Notify Compass providers of incarceration if the individual is in services </li> <li>Refer to CSS where appropriate (Rehab services, assistance with Disability if applicable, Housing, Employment)</li> <li>Refer to Care Coordination for additional follow up </li> <li>Work to ensure person incarcerated can remain on meds while in jail or get back on medication as soon as possible </li> <li>Mentoring through self-disclosure of personal recovery experience to assist with daily living skills, relationship building, identifying support networks and community resources, symptom self-management, and coping skills.</li> <li>Peer Support groups as arranged in the jail </li> <li>Assist in planning and implementing recovery group activities as assigned</li> <li>Work hours are flexible to meet the needs of the person served.</li> <li>Contacts with served individuals will be while the person is incarcerated and after care/referral to resources after discharge from jail </li> <li>Documentation of services in Credible </li> <li>Documentation in Jail data base as requested. </li> </ul> MINIMUM QUALIFICATIONS: <ul><li style="text-align:justify;">Must be at least 21years of age and have a high school diploma or equivalent. </li> <li style="text-align:justify;">Must self-identify as a present or former recipient of mental health or substance use services and be well-established in own recovery (minimum of 1 year).</li> <li style="text-align:justify;">Excellent interpersonal and teamwork skills </li> <li style="text-align:justify;">Excellent organizational and time management skills </li> <li style="text-align:justify;">Interest and investment in working with adults who are incarcerated in jail and need supports to manage in jail, make better choices/have good supports upon release from jail, and decrease incarceration recidivism. </li> <li style="text-align:justify;">Ability to form professional relationships with both Compass staff and County staff</li> <li style="text-align:justify;">Have completed or will complete any required training within six months of employment.</li> <li style="text-align:justify;">Must have a current and valid Kansas driver’s license</li> </ul> <img src="https://ars2.equest.com/? response_id=66567170ef78c59ef0dc435b16cf8ec5&view" alt=" " width="1" height="1"/>
