Responsibilities
• Build long-term relationships with patients and their families in an intimate outpatient setting • Deliver care to patients with multiple co-morbidities, requiring unique treatment plans and a broad range of nursing skills and knowledge • Oversee a group of patients with the support of PCTs to conduct patient observations, measuring stats and machine set up • Work in a fast-paced environment, using your head, heart, and hands each day

Requirements
• Associates Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from an accredited diploma program may be substituted for nursing degree • ICU, CCU, ER, or Med Surg experience may be helpful but is not required • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) certification may be beneficial
