The Registered Nurse Case Manager works with our Director of Nursing and Branch Manager out of our well-established BrightStar of St Croix Valley office and is responsible for providing care management services in a home care environment may be just what you are looking for.

The RN Case Manager is responsible for overseeing the care provided to our clients as directed by BrightStar Care and our Joint Commission accreditation quality and safety guidelines.

A typical day may include performing new client assessments, overseeing the care of existing clients, medication reconciliation, wound care, and care coordination with skilled service organizations and hospices. Compassion, patience and an outgoing personality are qualities that will build trust and a level of comfort with families. The RN Case Manager will work primarily in the field and rarely spend time in the main office. Travel in Washington County required. Most work is typically performed Monday through Friday 8 AM to 5PM. We also have very nice Company Cars for our nurses to use!

Responsibilities:

  • Provide thoughtful care to client & patients based on personalized Plan of Care
  • Monitor and track changes in the client/patients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
  • Proactively updates client/patients family on plan of care, health changes, and concerns

Requirements:

  • Current unrestricted RN license in Minnesota and or Wisconsin
  • Certified CPR and negative TB skin test or chest x-ray
  • Minimum of one (1) year of documented experience required. Two (2) or more years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred
  • Licensed driver with valid automobile insurance
  • Excellent Computer Skills, 100% Digital Charting
  • Infusion Experience is a Plus!
  • Excellent customer service skills
  • Treat clients, staff and the public with courtesy, respect and presents a positive public image
  • Case management skills a plus
  • Ability to travel including weekends, nights and holidays if necessary
  • Understands regulatory compliance such as OSHA and HIPAA
  • Experience with, or desire to, participate in marketing and sales a plus
  • Flexibility and good time management skills A MUST
  • Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
