Position Overview: The RN Clinical Care Coordinator coordinates and provides clinical case management services to high-risk patients within the Behavioral Health Community Partner (BHCP) program and ensures coordinated care and access to Behavioral Health services. The RN Care Coordinator participates in problem-solving by working as a closely integrated member of the Multidisciplinary Care Team within and across the Health Center. Additionally, the RN Clinical Care Coordinator performs Medication Reconciliations for all patients who have a completed a Comprehensive Health Assessment.
Duties:
• Approves all Comprehensive Risk Assessments.
• Provides clinical support to other programs and departments as appropriate.
• Works closely with providers to ensure assigned patients engage in primary care.
• Uses the stages of change model with patients to guide patients.
Qualifications
Minimum Basic Knowledge : Licensed Registered Nurse in Massachusetts
Experience & Qualifications :
  • Two years of nursing experience, preferably in an ambulatory care or community health setting preferred.
  • Experience with case management and care coordination.
  • Experience with EMR and related care management registries, databases and Information Technology/Computer Systems.
  • Demonstrated ability to work as an integrated member of a Multidisciplinary Care Team.
  • Excellent communication and clinical judgment skills.
  • Ability to follow and adhere to care model's operating procedures and standards.
  • Experience in a culturally diverse setting.
  • Bilingual language skills in English/Spanish, Portuguese or Vietnamese preferred.
New or recently graduated nurses are encouraged to apply!
Independent Action :
  • Functions independently within scope of nursing practice, the Primary Care Payment Reform Initiative's requirements and health center policies and goals.
  • Refers specific problems to supervisor when clarification of operating policies or procedures is required.
  • Completes outreach for patients who are inpatient or who have utilized the Emergency Room

Supervisory Responsibility :
There are no supervisory responsibilities.
