Job Summary
Provides medical care to patients in the long-term care setting. Patients will be of varying types and acuity and the Physician will be responsible for the continuous and comprehensive management of a broad range of health services.
Essential Duties and Responsibilities

  • Review and analyze patient medical records; including medical history and laboratory results to identify the need for surgery.
  • Assesses physical status of patient through physical examination.
  • Provide education and explanation to the patient and other significant parties regarding the nature of the procedure and the risks involved.
  • Maintain communication with long term care facilities regarding policies and procedures.
  • Evaluates outcomes of direct patient care and consults with other specialists as required.
  • Maintains required electronic medical records and records progress notes as to status, treatments and procedures. Perform timely, accurate and concise documentation of patient care.
  • Prescribe medical care and observe, investigate and manage the patients progress and care.
  • Coordinate patient care with other medical staff as needed.
  • Adheres to organizational and department safety standards and guidelines
  • Participate in the hospital's quality improvement program.
  • Demonstrates clear ownership of workplace and patient safety.
  • Reports mistakes, near misses, adverse events and quality and safety concerns.
  • Participates in the development and implementation of safety and quality improvement activities.
  • Protects patient privacy and confidentiality.
  • Other duties as may be assigned.

Job Requirements
Education : Graduate of an accredited medicalschool.
Licensure/Certification: Board certified. A valid license to practice in Indiana is required. Indiana driver's license in good standing.
Experience : Geriatric experience required. Strong communications skills, verbal and written, required. Must possess well-developed interpersonal skills to work smoothly and productively with patients, as well as administrative, nursing and physician staff members. Strong computer skills and use of electronic medical records required.
Physical Requirements : Frequent standing and walking; occasional sitting. Pushing, pulling, stooping, and bending. Occasionally lift up to 50 lbs.
