Bay State Physical Therapy is seeking a self motivated, ambitious Physical Therapist to join our team in East Bridgewater, MA. As a member of our team, you will provide one-on-one individualized, high quality care that is offered in a comfortable learning environment with a focus on educating our patients about their health and wellness.
• Our clinics are located within medical facilities, free standing out-patient clinics and within local fitness centers and YMCA's.

• Evaluate incoming patient conditions, develop and implement physical treatment programs and consult with referring Physicians regarding diagnosis, compose discharge notes for patients and refer to appropriate outside sources as necessary
• Assist patients with all phases of physical therapy treatment and techniques
• Document treatments, record patient conditions and responses to treatments
• Prepare patient care/progress notes and other required forms as requested.
• Operate and apply knowledge of standard physical therapy equipment such as ultrasound equipment, nautilus equipment and orthotron machines.

Responsibilities include but are not limited to:
