The Oncology Infusion Nurse Navigator is responsible for partnering with patients and providers to offer personalized, individualized care of patients throughout the continuum of care to increase and assure timely access to healthcare services. This role will support the East Region of Hartford Healthcare which will include Backus and Windham Campuses. Key Responsibilities include but are not limited to:

• Serving as a member of the multidisciplinary care team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.
• Navigating patients through the continuum to eliminate barriers to health care and enhancing patient and provider satisfaction.
• Applying the nursing process as appropriate within the context of the organization's framework for professional practice and in accordance with guidelines established by the team.
• Assessing patients for psychological distress and making appropriate referrals if more intensive support is needed.

Qualifications

Education: BS in Nursing strongly preferred; Bachelor's Degree Required

Licenses/Certifications: CT RN license required. Certification in specialty area preferred

Experience: Minimum 5 years clinical nursing experience preferred
