Overview
HarmonyCares is seeking an Occupational Therapist to join our home-based primary care team. We are dedicated to providing high-quality, coordinated health care in the home.
Responsibilities
- Assist in evaluating level of function and help develop treatment plans
- Guide and instruct patients in therapeutic and self-care activities
- Advise and instruct patients and families in the use of assistive devices
- Participate in Performance Improvement Activities and discharge planning
- Ensure Age-Friendly Care and use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility)
Qualifications
- Graduate of an occupational therapy program accredited by the appropriate bodies
- Currently licensed as an Occupational Therapist with at least 2 years of experience
- Active CPR Certification
- Valid driver's license, reliable vehicle, and good driving record
