MDS Coordinator (RN) - Artesia Palms Care Center

Responsibilities<p>• The MDS Coordinator completes MDS for all residents according to OBRA and PPS guidelines<p>• Assists in providing a clean, safe, happy and healthy environment for residents by performing the duties as described below<p>• The goal is to promote the patient's well-being by providing high quality nursing care<p>• Primary MDS Coordinator Responsibilities<p>• Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement<p>• Determine potential Resource Utilization Groups (RUGs) and expense associated with a potential admission<p>• Review all applications for admission, followed by on-site or phone assessment and communicate results of assessment to the Admissions Coordinator<p>• Complete and assure the accuracy of the MDS process for all residents<p>• Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations<p>• Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement<p>• Facilitate problem-solving for complicated admissions<p>• Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission<p>• Complies with the facility's attendance policy requiring regular, reliable, and punctual attendance<p>• Adjust and administer patient's medication and provide treatments according to physician's orders<p>• Ensures timely response to call lights<p>• Pain management/intervention<p>• Wound treatments<p>• Therapy treatments<p>• Incidents; falls, etc<p>• Patient complaints / concerns<p>• Identifies/assesses emergency medical situations and provides appropriate interventions as indicated<p>• Documentation and follow-up with timely notification of physician and family<p>• Care planning as needed<p>• Performs other duties or functions as assigned by the DON and/or Administrator <img src="https://ars2.equest.com/? response_id=f7335be003543a727addd69656905843&view" alt=" " width="1" height="1"/>
