Job Summary:

The Oncology Navigator will be responsible for care coordination with the patient, involved family members of the patient and multi-disciplinary team regarding the diagnosis, treatment, management, logistics of care and survivorship related to cancer. Will work effectively with staff, various departments and medical organizations to provide seamless care along the healthcare continuum. Assists care coordinators in the assurance of case management and patient care coordination. This will include ensuring scheduling of appointments, ensuring transportation needs, ensuring appointment attendance, ensuring patient and involved family members understand the diagnosis, treatment plan and while addressing questions and concerns. The Oncology Navigator will direct patients and their family members to local and national resources. Coordination of care will be through system created referral plans unless patient elects to deviate from the established provider team. Care Coordination for Oncology patients promotes delivery of quality care for oncology patients across the entire continuum of care in addition to cancer prevention and outreach activities. The Oncology Navigator must be able to maintain strict confidentiality when dealing with patient and hospital information. They should be organized and meticulous with a sharp eye for detail, a professional attitude, the ability to multitask and adapt to an ever changing demanding workload.

Job Relationships :

  1. Directly reports to Primary Care Practice Manager or designee.
  2. Works closely with various departments within NIHD and other medical organizations including, but not limited to Medical Oncology, Radiation Oncology, surgery and infusion centers, provider offices, etc.
  3. May report indirectly to other senior leadership responsible for specific function oversight.

Occupational Hazards:

Works in the general hospital setting. May be exposed to blood, body fluids, or tissues as well as patients in varying stages of infectious states. Travel between work sites in personal vehicle.

Samples of Duties:

  • Receive oncology navigation referrals from physicians’ offices and/or patients and
  • Coordinates with NIHD and other medical organizations to facilitate appointment needs
  • Answer and screen phone calls in a professional and courteous manner
  • Submit and monitor orders as requested by physicians or nurses
  • Participate in Quality Assessment activities as directed for the continuous improvement of patient care
  • Act as a resource and assists other members of the department as needed
  • Maintains an active role in problem identification and problem solving
  • Collects and evaluates data related to oncology statistics and outcomes
  • Provides educational information and resources related to patients and families in order to assist with coping, foster self-care activities and financial support.
  • Support Care Coordination team with ER follow-ups and other care coordination duties


Knowledge, Skills and Abilities:

Knowledge of:

  • The Mission, Vision, and Values of Northern Inyo Healthcare District;
  • NIHD policies and procedures, including Compliance Program, Code of Conduct, and Confidentiality standards;
  • Understand the function and work flow of the perioperative department and the services provided

Skills/Abilities to:

  • Apply critical thinking and problem solving skills to department and patient concerns including financial, authorization, and/or insurance issues
  • Communicate effectively both verbally and in writing with NIHD staff and the public;
  • Establish and maintain professional working relationships with NIHD staff at all levels as well as the public;

Minimum Qualifications:

Experience:

3 years of experience working in health care

Education:

High School Diploma or equivalent is required

LVN License

Certification:

BLS upon hire

Navigator Certification within a year

Four - 10 hour shifts per week
