Integrated Care Manager I, II, III

Two (2)/Full-Time/Non-Exempt/$19.95 - $24.15 per hour (DOE)

Seeking 2 Full-Time Employees:

1 working out of the Lakeport Campus (position works primarily with children and young adults);

1 working out of the Clearlake office (position works with people of all ages)


Pay differentials offered - must qualify through testing or verification for the following special competencies.

*Language Translation - Bi-cultural Spanish ($1.00 per hour) *Bimodal Bilingualism - ASL Sign Language - Interpret from American Sign Language (ASL) to English, and English to ASL. ($1.00 per hour)


The path to healthcare and improving your life is complicated and challenging and even more so for our vulnerable community members who experience homelessness, severe mental illness, substance use disorder, and/or serious medical challenges. As an Integrated Care Manager you provide support to these highly vulnerable participants through intensive coordination of health and health-related services. You will coordinate care and services among the physical, behavioral, dental, developmental, and social services delivery systems, making it easier for participants to get the right care at the right time. These services improve access to care for people with complex needs and who are facing difficult life and health circumstances. You will develop a comprehensive care plan with each participant and act as their primary point of contact to navigate the various system of care they need to access to meet their needs. Additionally, you will ensure all providers involved in the participant’s care are communicating with each other and working together. Requires at least one (1) year experience working in the social work field providing direct client services. Position has potential for a flexible schedule – some weekend or evening hours could be needed to meet client needs. This position requires compensated use of personal vehicle – must have CA driver’s license. Bilingual Spanish-speaking candidates strongly desired.

Experience:

  • Care Manager I: An individual who has less than two (2) years of FTE experience in the mental health-related field providing direct mental health services.
    • Minimum of one (1) year of experience working in a social work-related position providing direct client services. Prefer candidates with experience working with people experiencing homelessness.
  • Care Manager II – At least two (2) years of full-time experience in the social work field providing direct services         
  • Care Manager III – At least four (4) years of full-time experience in the social work field providing direct client services. Up to a maximum of two (2) years can be substituted by years of education.** Demonstrates ability to work independently with competency in all aspects of the role.**     
    • **Minimum of two (2) years of college level education (60 semester or 90 quarter units) with a minimum of 12 semester (18 quarter) units in social work-related subject areas such as child development, social work, human behavior, rehabilitation, psychology, or substance use counseling. Must be able to provide written verification of experience upon hire. 
  • Strongly desired  – Candidates with lived/personal experience with homelessness, mental illness, substance use disorder, and/or other challenges experienced by the people served.

Other Qualifications:

  • Position requires driving for the agency, including transporting clients. 2 years driving experience required with no more than three (3) points on driving record. Must be able to use personal vehicle (with compensation). Proof of vehicle insurance with 100/300 liability required.
  • Must have ability to occasionally or regularly work a flexible schedule, including weekends and evenings.
  • Strongly desired  – Candidates that speak and/or are fluent in Spanish or American Sign Language (ASL).

Training:

  • Preferred  – Training in harm reduction, Housing First, trauma-informed, low barrier service models.
  • RCS provides an in-depth onboarding and training orientation, which includes CPR/First Aid, Crisis Communication/Pro-ACT, Trauma-Informed Care, Motivational Interviewing, Collaborative Problem Solving, introductions to all RCS departments, and other valuable trainings.


CONDITIONS OF EMPLOYMENT: This position may work within ALL other agency programs, consistent with their scope of practice, on a as needed basis. In addition, may require occasional flexible work schedule to meet the individualized needs of each client.  


Certificates, Licenses, Registrations: CA Driver's License, Current Auto Insurance, Clean DMV Printout (No more than 3 points on driving record), Subsequent background check required should you be chosen as a candidate for this position. CPR and First Aid Certified. TB Test and Physical at the time of employment.
