Your Responsibilities:
- Determine patient care needs and carry out goal-oriented plans of care.
- Ensure accurate and timely documentation of patient information, adhering to HIPAA regulations.
- Work autonomously under RN direction and utilize supervisory support when necessary.
- Empower clients to problem-solve by exploring options of care, when available and appropriate, and alternative plans, when necessary, to achieve desired outcomes
- Participate in on-call rotation and respond to calls promptly.

- Valid Vermont LPN License
- Minimum of one year progressive clinical experience. Hospice experience preferred.
