Position Summary

As a member of the Home Health team ensures patients receive timely admission to home health. Creates a specialized plan of care in collaboration with patient and family with a focus on improving the health and safety of the patient. Provides evaluation, direction, and application of licensed nursing services to carry out the plan of care and treatment in the homes of clients under the direction/supervision of the Clinical Director and within the realm of accepted standards of practice for a RN as stated in the Nurse Practice Act for the State of practice.

Essential Duties and Responsibilities

The following statements describe the principal activities of this position identified and shall not be construed as a detailed exposition of all the duties that may be inherent in this position.

• Completes an initial comprehensive assessment of patient and family/caregiver to determine home health needs.

• Provides a complete physical assessment and history of current and previous illness(es) to determine nursing needs and any follow up needed;

• Initiates the plan of care;

• Assists in development, review, and revision of the plan of care in conjunction with patient, caregiver, representative, if any, identifies interventions which will allow patient and family/caregivers to meet their goals;

• Provides nursing care in accordance with physician’s ordered plan of treatment, individual plan of care, as authorized by client’s payer source;

• Provides ongoing instruction and supervision of client, family, caregiver, representative, if any or other health team personnel in the plan of treatment;

• Provides professional nursing care;

• Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care givenwithin 24 hours;

• Observes, records, and reports reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician;

• Administers medications and treatments as prescribed by the physician in the physician’s plan of care;

• Records pain/symptom management changes/outcomes as appropriate;

• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required;

• Assists the patient and family/caregiver and other team members in providing continuity of care;

• Ensures that supplies, medications and other necessary items and ancillary services are available and delivered/provided when needed;

• May supervise LPN’s and Home Health Aides;

• May take an on-call rotation;

• Prepares and submits timely written documentation per nursing practice standards in compliance with regulatory requirements;

• Participates in education programs, quality assessment activities, nurse’s meetings and other staff meetings as required;

• Respects the uniqueness of living conditions and family dynamics of patients admitted to home health services;

• Complies with Compliance Plan;

• Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient’s care;

• As business needs may change, may perform additional responsibilities assigned by supervisor that are reasonably related to the position.

Education and Experience Requirements Education:

Education:

• Graduate of approved School of Professional Nursing;

• Current RN licensure for the State of practice;

• Possess and maintain a current CPR certification.

Experience:

• Home Health experience preferred but not required.
