Our Company is seeking the right individual to become the Clinical Director of our Home Health department. We believe our clients deserve to have a safe home life with dignity and independence. We need true leaders of Home Health to help us meet the needs of our clients and achieve our goals. 

The Clinical Director will oversee all Home Health operations and will perform the following daily tasks, but is not limited to...

  • Managing the quality, cost and compliance aspects for our team of clinicians and respective patients
  • Planning and facilitating weekly Clinical Case Conferences
  • Conducting staff meetings, providing education re: clinical resources, HH regulations, and IHCH policies and procedures
  • Resolving patient issues and concerns, as needed
  • Orienting and mentoring assigned clinicians and support staff
  • Collaboration with our support departments to identify opportunities and implement improvement
  • Conducting field supervisory visits (RN only).
  • Assuring preparation/follow-up of call schedule.
  • Assists with HR functions, including interviewing, hiring, performance evaluations and payroll for assigned staff members.

Education:

  • Be a RN Graduate of approved School of Nursing 
  • Posses current state license in respective discipline

Experience Requirements

  • Six (6) months experience as a practical/vocational nurse in a hospital, nursing home or home care setting within the last three (3) years is preferred. 
  • At least 18 years of age
  • Posses minimum of one (1) year experience in care coordination
  • Must have experience with Oasis Assessments
  • Supervision and administrative experience in health care setting is preferred
  • 6 months experience caring for the elderly or disabled
  • Reliable transportation
  • driver's license
  • proof of current auto insurance
