Read below on how you can make a difference in our community and apply today!</p> <p><span style="color:#c83c1b;"><strong>SCOPE OF ROLE</strong></span></p> <p>Reporting to the Manager, Behavioral Health Community Liaison, the Counselor, Hospital Liaison will serve as the contact person responsible for coordination of care and discharge planning for newly referred and existing Lake County patients admitted to inpatient treatment. The Counselor, Hospital Liaison will diligently advocate for the client by providing treatment support between the hospital and client.</p> <p><span style="color:#c83c1b;"><strong>HOW YOU’LL SUCCEED:</strong></span></p> <ul><li>Ability to travel to non-Signature Health, Psychiatric and SUD facilities as determined by patient needs.</li> <li>Conduct diagnostic assessments on new patients referred by hospitals, either at the hospital or in the community/office.</li> <li>Take our organization to new heights by engaging in hospital trainings that cover specialized applicable care such as forensics, legal, or hospital systems that directly impact the quality and type of care offered.</li> <li>Maintain patient charting in accordance with Medicaid, JCAHO, OMHAS, CARF and professional standards.</li> <li>Act as an outstanding point of contact and advocate for all clients that are hospitalized.</li> <li>Attend weekly meetings at assigned site to update hospital personnel on company efforts to transition patients into the community once discharge ready. </li> <li>Participate in coordination of patient care through review of treatment history and communication of resources.</li> <li>Maintain regular contact with hospital staff as needed.</li> <li>Properly report and document client interactions, findings, information, recommendations and efforts/linkages in the patient’s hospital chart to hospital personnel or social worker.</li> <li>Update and encourage participation in the treatment planning and provision of care supporting effective discharge.</li> <li>Facilitate follow up appointments with patient’s existing or new provider for continuity of care and prevention of re-hospitalization.</li> <li>Attend any ADAMHS Board trainings or meetings regarding the liaison role. Report and present back to wider company clinicians.</li> <li>Increase frequency of services as consumers approach discharge to ensure suitable housing, active benefits and entitlements, and follow up appointments.</li> <li>Maintain positive relationships with patient guardians and/or family members who are essential to current care and support upon client discharge.</li> <li>Provide direct services to clients by establishing a strong rapport.</li> <li>Other duties as assigned.</li> </ul> <p> </p> <p><span style="color:#c83c1b;"><strong>KNOWLEDGE &amp; EXPERIENCE:</strong></span></p> <ul><li>Bachelor’s Degree required. Master’s Degree in social work preferred.</li> <li>Valid unencumbered LSW, LISW, LPC, LPCC, LIMFT or LMFT License in the state of Ohio required.</li> <li>1-2+ years of assessment skills experience preferred.</li> <li>Computer Skills: Proficient in Microsoft Office and Electronic Health Records. Knowledge of word processing software.</li> </ul> <p><span style="color:#c83c1b;"><strong>WORKING CONDITIONS:</strong></span></p> <ul><li>Work is normally performed in a typical interior/office/clinical work environment.</li> <li>While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.</li> <li>Requires periods of sitting, standing, telephone, and computer work. </li> <li>Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.</li> <li>Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.</li> <li>Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.</li> <li>Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.</li> <li>Possible exposure to blood borne pathogens while performing job duties.</li> <li>Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.</li> <li>Sufficient dexterity to operate a PC and other office equipment.</li> </ul> <p> </p> <p>This Success Profile is not an exhaustive list of all functions or requirements that you may be required to perform; you may be required to perform other job-related assignments as requested by your supervisor or the company.
