<p><b>Compensation</b>: $24.04/hour; $25.31/hour QMHP</p><p></p><p><b>Due to rapid expansion in all of our clinics, we are offering a $1,500 sign on bonus for Community Support Specialists; additional $500 sign-on bonus for Qualified Mental Health Professionals. We are eager to be able to continue to serve clients in our service areas and need you to make this happen!</b></p><p><b>This is a full-time, benefit eligible position.</b></p><p>The Community Support Specialist Youth assists eligible children and their families to achieve or maintain optimum social, psychological, and physical functioning. This is to be done through the planning, securing, coordinating and monitoring of services from different organizations and personnel on behalf of those served and the teaching, coaching and practicing of necessary skills to facilitate community based psychiatric rehabilitation.</p><p>CHN team members enjoy a collaborative team environment, and competitive benefits, including quarterly performance incentives.</p><p><b>Responsibilities of the Community Support Specialist</b></p><ul><li>Provide wraparound assessment for service planning</li><li>Assist in and monitor treatment plan development and complete necessary quarterly reviews</li><li>Maintain all assigned client medical records to program standards and complete documentation in a timely manner (within 72 hours)</li><li>Provide contact with families at minimum of twice per month based upon client/family needs, one at least face-to-face in the home<ul><li>If the home is not a safe environment then at another identified location</li></ul></li><li>Provide referrals, advocacy, information and education to meet individual needs of clients</li><li>Monitor families' progress and services they receive from various organizations</li><li>Maintain communication between the family and service providers</li></ul><p>* Driving is an essential function of the position</p><p></p><p> </p><br><br><p>High School/GED required<br />Associate's/Bachelor's/Master's degree in a human service-related field preferred</p><p><b>If no degree is obtained, one of the following is required:</b></p><p>At least four (4) years of experience OR a combination of higher education and experience required if a High School/GED is the highest education obtained </p><p>At least two (2) years of experience OR a combination of higher education and experience required if an Associate's degree is the highest education obtained <br />(*If the degree is an Associate's of Applied Science in Behavioral Health Support from an approved institution; then no additional experience is required)</p><p>At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained</p><p>All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities</p><p>A valid driver's license and agency established minimum automobile coverage required</p><p><b>Compass Health Network</b> is a nonprofit health care organization offering accessible, comprehensive, & compassionate behavioral health, substance use treatment, family medicine and dental services throughout Missouri.
