We are looking for a Community Health Worker (CHW) to join our team. A CHW has a particularly good understanding of the community served, acting as a liaison between health and social services and the community to facilitate access to services and to improve the quality and cultural competence of service delivery.

In this role, you will serve approximately 200 patients, extending the services of our providers by addressing issues related to social determinants of health. Working closely with Senior Health Express (SHE), you will provide a variety of services to our patients.

This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

Responsibilities:

  • Coordinate with the MSP/HC care team to ensure patient compliance with the care plan.
  • Deliver medications to patients and review instructions with them.
  • Perform medication reconciliation, comparing the patient's current medication list with the prescription and over-the-counter medications found in their home.
  • Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, and primary care providers.
  • Set up and instruct patients on remote patient monitoring equipment.
  • Make recommendations and training on medication administration alternatives such as LIVI.
  • Work with Health Coaches to relay information regarding patient issues in regards to meeting the goals of the patient's care plan.
  • Facilitating visits via telehealth with the providers and patients in the comfort of the patients' home.

Qualifications:

  • Certified Medical Assistant, paramedic, or nurse with relevant experience in community health or home health (Preferred)
  • Excellent interpersonal and communication skills.
  • Ability to work independently with minimal supervision.
  • Must have valid driver’s license and reliable transportation.
