Job Description
Dynamic new care coordination position providing care and support to youth and their families involved in Department of Mental Health Children Services!
The Intensive Services Coordinator will be part of the IHB team working with families experiencing mental health needs that impact their home and relationships. Position requires the ability to manage complex and competing demands in the office and in the community on a daily basis. The Intensive Services Coordinator will provide quality care for youth in need of stabilization and therapeutic services. S/he will accept and display the values of caring, honesty, respect and responsibility.

JOB FUNCTIONS:
1. PROGRAM/DEPARTMENT RESPSONSIBILITIES:
· Leads the IHB Team with intakes, orientation, treatment planning, and service delivery of the Family Team.
· Works in conjunction with the IHB team to develop and implement the treatment and safety plan.
  • Works closely with the Family Partner to support the Family Team.
  • Assist with developing and implementing creative solutions for youth to overcome barriers to success through role play, skill building and communication strategies.
  • Communicate with youth, families and collaterals daily in order to best meet their needs/goals.
· Attend family team meetings, case conferences, case coordination meetings as needed.
· Maintain all case records in a confidential and secure manner.
· Attends IHB staff meetings weekly.
· Conducts a minimum of weekly contact with youth/family through home visits and telephone.
· Facilitates and supports all assessments through goal setting around safety, skill development, strengths, needs, and identifying connections.
· Maintains professional boundaries at all times.
· Attends and actively participates in weekly individual/group supervision to review families’ progress, identify risk factors, improve communication and work on professional growth.
· Completes daily progress notes, treatment goals and other required documentation, including data entry, in a timely manner, adhering to the highest professional standards.
· Assists with and helps coordinate needs for crisis response and respite needs.
· Works a flexible schedule with on call responsibility.

Requirements
  • Master's Degree in Social Work (clinical license preferred) with two years of experience working with the target population.
  • Excellent communication (written and verbal), organizational and interpersonal skills
  • Demonstrate cultural humility with an ability to work with diverse populations, within the community and with agency staff members.
  • Possess basic computer skills for purposes of word processing and data entry.
  • Flexibility and ability to respond to families under stress, and in crisis, in a fast paced work environment.
  • Possess a current and valid driver's license, and automobile - must be able to drive daily to transport youth and attend meetings and home visits.
  • Must be able to walk, sit and stand during the work day
