Description

Sunnyhill is a not-for-profit organization established in 1978 that provides residential support, recreation and educational opportunities for children and adults with developmental and other disabilities throughout the Greater St. Louis Area and surrounding counties. Residential services are community based and promote a person's dreams and independence.

Lead Staff are continually involved in the assessment, development, and/or management of an individual's Support Plan. Lead Staff support individuals by coordinating services and facilitating opportunities that include options, choices, independence, empowerment, and community inclusion. Lead Staff have a demonstrated commitment to the vision, mission, values, goals and objectives of the Agency.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Provide consumer care and support in accordance with the Individualized Support Plan [ISP's], Behavior Support Plans [BSP's], staffing patterns, and budgets
  • Assist Home Manager with developing and maintaining staff schedules
  • Must work 40 hours per week, and shifts will typically be opposite of the Home Manager. In addition, will be on call 2 weekends a month from 5pm Friday- 9am Monday (shared with Home Manager) and when the Home Manager is on PTO. Sends out group texts on Friday and Monday to communicate to all team members who is on call.
  • Communicate positively and effectively with individuals Guardian's, Case Managers, and other treatment providers in the community
  • Work with team members to complete personal outcomes. Complete accurate and relevant log notes that addresses individual personal outcomes
  • Provide personal assistance in hygiene, toileting, bathing, eating and other activities of daily living as each individual needs it or as outlined in the individual ISPs
  • Follow cleaning and maintenance schedule to ensure home is well-kept
  • Administer all medications and medical treatments accurately and document accordingly.
  • Monitor, document, and adhere to the nutritional/dietary needs of consumers

Requirements

  • High School Diploma or GED
  • 1 or more years in the Human Service field preferred
  • Other requirements: Use of a personal motor vehicle, valid driver's license, adequate auto insurance as defined by state law. Acceptable police, DFS, and DMV record checks. Must successfully complete all company provided training.
