We are seeking Acute Care Advanced Practice Providers (NP/PA) to join our team. These providers will work in-home providing comprehensive health assessments, diagnosis, treatment, and outcome management to patients of all ages in the Sacramento area. Our model allows for point of care laboratory studies, minor procedures, wound care, suturing, IV administration, and more. This role will involve driving or riding in a company vehicle to deliver care to patients.
Responsibilities:
- Perform comprehensive health assessments and diagnose/treat complex illnesses.
- Provide therapeutic interventions and minor procedures.
- Use critical thinking skills and follow evidence-based practice.
- Accurately document patient encounters.
- Analyze test data and arrange appropriate follow-up care.
- Educate patients and families on illness management.
- Communicate effectively with patients, families, and other healthcare providers.
- Maintain clinical and safety standards.
- Attend training sessions and clinical team meetings.
- Maintain a professional demeanor as a representative of the organization.
Requirements:
- Minimum of two years of experience in ER, Urgent Care, or Internal Medicine.
- Graduate degree from an accredited institution.
- Current unrestricted state license as a NP or PA.
- Nationally board certified, current BLS required.
- Prescriptive authority and DEA.
- Valid driver's license with a clean driving record.
- Ability to lift and carry equipment up to 50 pounds.
- Flexibility to work evenings, weekends, and holidays.
