What You'll Do as a Registered Nurse:
- Conduct patient assessments and coordinate interdisciplinary care
- Initiate communication with physicians and other agencies for optimal care
- Update and implement care plans
- Perform necessary nursing procedures as directed
- Maintain accurate patient records and prepare care plans
What You'll Do as an LPN:
- Provide direct patient care
- Implement plan of care initiated by the registered nurse
- Provide accurate and timely documentation
- Participate in coordination of home health services
- Use equipment and supplies effectively and efficiently
- Participate in personal and professional growth and development
Qualifications:
- Registered Nurse:
- Graduate from an accredited nursing program and licensed in the state
- At least one (1) year clinical practical nursing experience
- LPN:
- Graduate of an accredited practical/vocational nursing program
- Three (3) years of nursing experience
- Currently licensed as an LPN/LVN in the state
- Home health experience preferred
- Valid CPR certification
- Reliable transportation and current driver's license
