Position Summary:

The Wraparound Facilitator is the member of the care team that will engage high-need youth and their families in developing, implementing, and monitoring an intensive, individualized care plan. This includes helping the care team develop a natural support system, working on the development of an integrated plan and establishing a strong relationship with the family. This partnership is important as the youth and family work to address challenges and make changes to their lives. The Wraparound Facilitator helps the family develop a positive view of the future through various activities, identifying and promoting resources available for the youth and family in their home community while safeguarding the process to ensure that the principles and steps of the Wraparound process are delivered with the highest possible fidelity to the model.

Essential Functions:

  • Model and act in accordance with BCC’s core values: safety, empathy, relationships, and impact.

  • Coordinate initial meetings with youth and family, ensuring the completion of intake documentation and necessary assessment tools in the identification of strengths, service needs, and transition plan.
  • Facilitate the development of the Wraparound care plan for each youth and family ensuring that the plan demonstrates youth and family voice and includes clear needs statements, strength-based strategies, and outcomes that are measurable and specific.
  • Collaborate with Wraparound care team to develop a crisis and safety plan to ensure the safety of the youth, family, and community.
  • Coordinate and facilitate ongoing child and family team meetings.
  • Meet agency productivity and documentation expectations with regards to weekly meaningful contact with youth and families.
  • Meet face-to-face with clients in their home or community for the provision of services.
  • Continuously reassess the needs of the youth and family with the care team and modify the care plan as needed, ensuring all members of the Wraparound care team are updated in a timely manner regarding plan modifications.
  • Evaluate the youth and family’s progress towards goal achievement and keep all parties informed with regards to progress toward achieving care plan goals.
  • Motivate and support family involvement through the identification and inclusion of informal supports.
  • Facilitates youth and family efforts to obtain any financial assistance or services for which they may be eligible under federal, state, and local regulations.
  • Seeks community resources first with the assistance of the team and modifies the care plan whenever services or resources need to be added and/or deleted.
  • Maintain effective working relationships with families, children/youth, colleagues, consultants, and community agencies to include scheduling home visits, notifications of serious incidents, follow-up on all ongoing documentation needs.
  • Complete required documentation and reports within the defined timelines including case notes, state and agency data collection, financial and programmatic reporting, assessments, care plan documentation, transition/discharge planning and post-discharge follow-up.
  • Provides or secures support and crisis/emergency services for the youth and family through face-to-face contact, phone contact or availability by an on-call system. Provide 24 hour on call support on a rotational basis.
  • Identify supervision and/or professional training needs to supervisor as they arise.
  • Demonstrate the ability to effectively express ideas clearly and concisely, orally and in writing.
  • Demonstrate the ability to engage youth and families in a positive therapeutic alliance.

Non-Essential

  • Assist with transportation needs of youth and family as needed.
  • Obtain and maintain certification as a “user” in BCC’s electronic health record software system

Work Environment:

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

In the nature of being at a residential and school living environment there may be times you experience conditions in which challenging behavior can be exhibited from our youth to include but not limited to, physical agitation, physical violence and/or emotion outburst. This position will require that you enter the homes and communities of the youth and families that we serve, you will need to have knowledge of safe home visiting practices.

The employee must occasionally lift or move up to 25 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Position Type/Expected Hours of Work:

This is a full-time position. Typical office hours are 8:30 a.m to 5:00 p.m., though the schedule requires flexibility based on the availability of youth and families served. This position regularly requires long hours and occasional weekend work. This position is eligible for flex schedule per BCC policy and procedures. This position is essential during inclement weather for on-call support.

Travel

Travel is primarily local during work, although some out-of-area and overnight travel may be expected by personal vehicle and/or agency vehicle dependent upon supervisor approval. Frequent local travel will be required within the home communities of the youth and families served. Talent will follow company procedures for work-related travel and expenses.

Minimum Required Qualifications:

  • Bachelors of Arts (BA) or Bachelors of Science (BS) in a human service or related field from an accredited college or university

Preferred Education and Experience

  • Master’s Degree in a human service related field from an accredited college or university, with professional licensure.
  • Documented experience, training or credentialing in Wraparound programming.
  • Experience working with or living with youth that have complex emotional needs.
  • A solid knowledge of community resources and different child-serving systems.

Additional Eligibility Qualifications

  • Must pass new hire orientation successfully and maintain up to date training hours in compliance as required and thereafter
  • Ability to effectively express ideas clearly and concisely, orally ad in writing; computer literate.
  • Able to work cooperatively with others.
  • Able to learn and effectively utilize different technology platforms.
  • Excellent computer use knowledge with Microsoft Windows 10 and 365 Office products, including Word, Excel, PowerPoint, and Outlook.

  • Ability to demonstrate the possession of a valid driver’s license prior to employment and ability to maintain the license with less than 7 points and operate a motor vehicle thereafter.
  • Ability to pass CPS/UII, and criminal background checks prior to employment, and as required. Ability to pass a Tuberculosis Screening and Pre-Hire Drug screening prior to employment and as required.
  • Ability to pass a work-readiness physical performed by a certified medical professional both prior to employment and as required.
  • Must be 21 years or older prior to employment.
  • Maintenance of any required license or certification of position, both at the time of hire and/or ongoing thereafter
