Home Visiting Nurse Practitioner
Responsibilities : Manage the transitional and chronic care needs of homebound patients including:

  • Assessment and Evaluation of current condition
  • Take past medical history
  • Review symptoms
  • Conduct general physical examinations
  • Review and reconcile medications
  • Screen for depression
  • Screen for alcohol and drug abuse
  • Identify diagnoses to be used in care management and active medical management for purposes of treatment
  • Recommend appropriate measures, including medication prescriptions and skilled needs for the patient
  • Efficiently communicate findings in your assessment to the office / supervising physician
  • Educate patients on topics such as disease process and prevention, medication, and compliance
Qualifications :
  • Current NP license, with no sanctions, restrictions, or provisions, in the state of Georgia
  • Russian Speaking
  • At least 1 year of clinical experience
  • Clinical background in adult, family or geriatric specialties
  • Experience working with an Electronic health record (EHR)
  • Effective communication skills are critical
  • Ease of use with smartphone and email are required
  • Reliable transportation
  • Ability to work independently
  • Home care or home visiting experience (preferred)
  • Willingness to spend at least 30 minutes to 1.5 hours with a patient in their homes, which may or may not be in the presence of pets, and/or family members that are tobacco users
