Remote Assessment Nurse

We are seeking a dedicated and experienced Virtual Assessment Nurse to join our team. In this remote role, you will conduct virtual assessments for members residing in downstate New York, including the five boroughs, Nassau, Suffolk, Rockland, Orange, Sullivan County, Long Island, and Westchester.

Location: This is a fully remote opportunity (Must reside in Bronx, Manhattan, New Rochelle or Yonkers, New York, and possess a NY RN License).

As a Virtual Assessment Nurse, you will ensure compliance with Department of Health (DOH) regulations and complete documentation to determine members' enrollment, service needs, and continued eligibility. Collaboration with the Care Management Team (CMT) is essential to maintain efficient operations of our long-term care plans.

Responsibilities:

Conduct Virtual Assessments: Hold telephonic and virtual meetings to conduct initial and re-assessments using the Uniform Assessment System (UAS) or other approved tools. Utilize clinical expertise and critical thinking to determine enrollment and eligibility. Conduct assessments in settings like skilled nursing facilities if needed.

Evaluate members' needs at the request of the member, caregiver, or CMT due to changes in condition. Review previous assessments and medical notes to adjust Care Plans and assist with nursing home placement eligibility.

Act as a liaison between the member and CMT, assessing home environments and psychosocial status. Coordinate with home care agencies, vendors, pharmacies, medical providers, and the CMT to address identified needs.

Provide feedback to home care agencies and the CMT regarding home care issues and the performance of contract nurses and aides. Educate the frail population on self-care techniques and preventative care strategies.

Ensure prospective members and their significant others understand and agree with enrollment into the managed long-term care plan. Perform additional duties as assigned, including care management tasks as needed.

Requirements:

Must have one year of UAS experience.

Valid New York State RN license.

Valid NY state driver's license or NYS DMV ID.

Experience in time management, critical/creative thinking, communication, and problem-solving skills.

Ability to work independently and handle high-volume caseloads.

Preferred Qualifications:

Previous field-based experience in assessing, planning, and evaluating members' care, preferably through Community Home Health Agency (CHHA) visits or similar roles.

Work experience in Geriatrics, Home Care, Discharge Planning, Case Management, or Medical-Surgical Nursing.

Experience with electronic patient health information (PHI) databases.

Knowledge of health insurance, Medicaid, Medicare, and Managed Long Term Care Plans (MLTCP).

Experience working with frail adults or elderly populations, determining appropriate care plans and services, and negotiating initial service plans with members and families.

Language proficiency in Spanish, Russian, French, Creole, Mandarin, or Cantonese.

Intermediate proficiency in Microsoft Word, Excel, and Outlook.

Pay Range:

$50-$56/HR depending on experience.

Hours:

Monday- Friday daytime hours, 40-hour week.
