Your responsibilities will include:- Patient Engagement: Conducting thorough intake assessments and preparing patient histories.- Data Management: Effectively managing lab data and providing essential follow-up steps to Education & Support: Educating patients and their families on health matters, while guiding them through their healthcare journey.- Advanced Triage: Performing advanced telephone triage functions to prioritize patient care effectively.- Collaboration: Serving as a liaison between patients, families, and our healthcare team, fostering a supportive environment.- Documentation: Maintaining accurate medical records and documentation to ensure seamless care.- Quality Assurance: Participating in QA functions and ensuring the highest standards of care.- Training & Orientation: Assisting with the orientation and training of new personnel to help us grow our team.Hard Skills:- Valid RN License- Patient Triage Expertise- Medicine Management ProficiencySoft Skills:- Exceptional Communication Skills- Strong Attention to Detail- Empathetic Approach to Patient Care- Collaborative Team PlayerPosition Details:- Job Type: Contract-to-Hire (Duration: 6 Months)- Work Location: Fully On-Site in Kingston, New YorkWork Environment:- Schedule: Monday - Friday, approximately 8 AM - 5 PM (with flexibility to extend until 6 PM if needed).- Locations: Multiple openings available in Putnam County, Green County (The Catskills), Ellenville, Accord area, Ulster County, Kingston, and Cottekill, NY.Compensation:- Competitive hourly pay rates range from $31.57 - $37.98, based on location and experience.Why Join Us?
