Job Summary:
ATN Healthcare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Health team. The LPN will provide quality nursing care to a home health client, ensuring their health, safety, and well-being. The ideal candidate will have experience in home health care and a passion for improving the lives of patients.
Responsibilities:
  • Patient Care: Provide direct nursing care to the client according to the care plan developed by the registered nurse and physician.
  • Assessment: Conduct regular assessments of the client's health status and report any changes to the healthcare team.
  • Medication Management: Administer medications as prescribed and monitor for side effects or adverse reactions.
  • Wound Care: Perform wound care and dressings as needed.

  • Mobility
  • Vital Signs Monitoring: Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature.
  • Documentation: Maintain accurate and detailed records of patient care, observations, and treatments.
  • Education: Educate the client and their family on healthcare practices, self-care techniques, and disease management.
  • Communication: Communicate effectively with the healthcare team, client, and family members to ensure coordinated and continuous care.
  • Compliance: Adhere to all federal, state, and local regulations and guidelines, as well as the policies and procedures of ATN Healthcare.
  • Supervise CNAs, Home Health Aids, and care givers.
Qualifications:
  • Licensure: Current and valid LPN license in the state of Colorado.
  • Education: Graduation from an accredited LPN program.
  • Experience: Minimum of one year of experience in home health care preferred.
  • Skills:
    • Strong clinical and assessment skills.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Proficient in electronic health records (EHR) and documentation.
  • Certifications: Current CPR certification.
  • Transportation: Reliable transportation and valid driver's license.
Physical Requirements:
  • Ability to lift and move patients as required.
  • Prolonged periods of standing, walking, and moving.
  • Ability to work in various home environments.
Work Schedule:
  • Flexible scheduling with the potential for weekends, holidays, and on-call duties as needed.
