PROGRAM DESCRIPTION:

The

San Mateo County Pride Center

is the first of its kind in San Mateo County and provides intensive support services to individuals and families in the LGBTQ+ community. The Pride Center is a partnership between StarVista and Outlet of Adolescent Counseling Services, in collaboration with San Mateo County Behavioral Health and Recovery Services. The Pride Center provides a broad range of services with three main components: 1) Clinical- individual, group, couples and family counseling, case management; 2) Social/Community -event and gathering space, peer support groups, education, trainings; and 3) Resource center -library, referrals, web-based media center, gender-affirming closet and more. POSITION PURPOSE AND OBJECTIVES:

The LGBTQ+ Case Manager will provide affirming, strength-based holistic case management services to LGBTQ+ individuals and families. Most of the clients served will be from marginalized, stigmatized, and vulnerable populations and may have co-occurring disorders and trauma. The LGBTQ+ Case Manager will use an intersectional lens to conduct thorough assessments of clients' needs and connect clients to appropriate local community resources to support their health and wellbeing. The LGBTQ+ Case Manager will also support in processing incoming referrals and client inquiries, including but not limited to scheduling and conducting initial screenings of clients requesting therapy and/or case management support. In this manner, we can better ensure that Pride Center clients can receive wraparound care upon first contact and more easily access the resources, providers, and/or services they may need.

Case management is a collaborative process that involves the coordination of community-based services and resources to provide support to individuals, partners, and families in need. The goal of the case management program is to enhance participants' abilities to resolve issues, overcome obstacles, and lead fulfilling, productive lives. The LGBTQ+ Case Manager works closely with clients to create a client-centered, customized service plan that considers each person's unique strengths, needs, and circumstances.

Requirements

ESSENTIAL JOB FUNCTIONS: Provide affirming, strengths-based, holistic case management services to marginalized and vulnerable LGBTQ+ individuals across San Mateo County. Connect clients and community members to appropriate and culturally relevant health, wellness, housing, employment, legal, education, and economic resources and/or services provided by the Center and other entities throughout San Mateo County and the surrounding area. Develop and implement individualized service plans for each case management client. Assess needs and strengths of clients on caseload; monitor clients' progress towards goals; and adapt service plans as needed. Provide ongoing care to clients and follow up depending on clients' needs. Work collaboratively with Pride Center clinical team to support overall health and wellbeing of Pride Center clients, including providing case management and/or resources to Pride Center therapy clients as needed. Process incoming referrals and coordinate intake requests for clinical services (including therapy and case management). This includes scheduling and conducting initial screenings of clients and helping to determine appropriate Pride Center clinical team provider(s) to meet clients' needs. Coordinate and facilitate monthly Legal Name and Gender Change workshops to assist community members with the process of legally updating their name and gender markers on their identity documents (e.g. driver's license, birth certificate, etc.). Training will be provided. Maintain and update Pride Center LGBTQ+ resource collections of affirming service providers, organizations, and businesses to ensure listings are accurate, up-to-date, and easily accessible. Comply with data collection requirements and ensure accurate, quality documentation and data entry for services provided, assessments, contacts, and internal/external referrals made. Participate in overall program evaluation efforts in collaboration with Pride Center Clinical team, Pride Center Manager, and StarVista Database team to support with reporting requirements, overall program evaluation, and grant proposals as relevant. Provide guidance and support to Case Management Interns and/or volunteers as needed. Participate in staff meetings and trainings as well as organizational retreats and gatherings. Attend weekly clinical program team meetings, clinical group supervision, and meetings with assigned supervisor for check-ins, support, and guidance. Build and sustain meaningful relations with clients, community members, and external partners. Engage and motivate community members to participate in Pride Center programs and services. Attend professional development trainings focused on LGBTQ+ issues and community meetings. Function as a core member of the San Mateo County LGBTQ+ Pride Center team to most effectively meet the needs of the LGBTQ+ community. Perform other duties as needed. KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

Understanding of issues impacting the LGBTQ+ community including the high rates of housing/economic insecurity, health inequities, suicide, mental health issues, and substance use. Strong understanding of anti-oppression, anti-racism, intersectionality, social justice, and cultural humility. Proficiency in trauma-informed, strength-based interventions, active listening, motivational interviewing skills, and crisis de-escalation techniques strongly preferred. Familiarity with San Mateo County and Bay Area community LGBTQ+ health and wellness resources and service providers a plus. Bi-lingual in Spanish, Tagalog, Chinese (Mandarin, Cantonese), and/or Tongan preferred. MINIMUM QUALIFICATIONS:

2+ years previous experience as a Case Manager or similar role highly desired. Lived experience in the LGBTQ+ community and/or prior experience and training working with LGBTQ+ individuals (especially those who are transgender, gender-diverse, or non-binary) highly desired. Prior experience working with historically marginalized populations strongly preferred: including those who are low- income, housing/food insecure, BIPOC, youth, older adults, disabled, immigrants and/or those who have a history of trauma and behavioral health challenges. Experience working in nonprofit community-based mental health and/or community centers a plus. Ability to work in a multidisciplinary, multiracial, multi-gendered, and intergenerational environment. Strong personal organization and communication skills. Strong commitment to social, racial, and economic justice. Bachelor's degree in social work, psychology, counseling, or equivalent experience preferred. Proficiency in Microsoft Office (Word, Excel, SharePoint, Outlook, etc.) and virtual meeting platforms like Zoom/Microsoft Teams preferred. Willingness to travel throughout San Mateo County (mileage reimbursement is provided). Valid CA Driver's license, working car, proof of auto insurance, and ability to pass fingerprint and TB clearance is required. BENEFITS:
