General Summary:
Under supervision, and in collaboration with the physician staff, the Lactation Nurseis primarily responsible for providing lactation consultation and education for breastfeeding mothers in the outpatient setting at Marin Breast Surgery and Breastfeeding Medicine clinic. The Lactation Nurse has excellent assessment and implementation skills to assist the breastfeeding mother attain her individual goals. The Lactation Nurse functions as a member of a multidisciplinary health care team as a consultant, educator, and mentor. The Lactation Nurse also acts as a role model for the medical staffs and the patients. Employees in this position must be able to demonstrate the knowledge and skills necessary to provide care and/or service based on the physical, psycho/social, educational, safety, and related criteria appropriate to the age of the patients served in his/her assigned service area. The Lactation Nurse will primarily be serving newborn infants and women of childbearing age including adolescent through middle adulthood. Interface with families requires that an understanding of the dynamics between child and parent be present.
This Position is secondarily responsible for both clinical and case management responsibilities with patients. The Nurse provides patient assessment to triage OB/GYN patients for appropriate follow-up actions. Further, the Nurse is responsible for the ongoing monitoring of clients with high-risk conditions.
Essential Functions and Responsibilities:
• Demonstrates clinical expertise by serving as a clinical expert, role model, and clinical and educational resource to the team, patients, and families
• Consistently demonstrates the ability to transfer scientific knowledge from social, biological and physical sciences in applying the nursing process as follows:
o Formulates a diagnosis in collaboration with the physician through observation of the patient's physical condition and behavior, and through interpretation of information obtained from the client and others, including the health team
o Formulates an age-specific care plan in collaboration with the physician and patient, which ensures that direct and indirect nursing care services provide for the patient's safety, comfort, hygiene, and protection, and for disease prevention and restorative measures
o Perform skills essential to the kind of nursing action to be taken, explains the health treatment to the patient and family and teaches the client and family how to care for the health needs
o Delegates tasks to subordinates based on the legal scopes of practice of the subordinates and on the preparation and capability needed in the tasks to be delegated, and effectively supervises nursing care being given by subordinates
o Evaluates the effectiveness of the care plan through observation of the patient's physical condition and behavior, signs and symptoms of illness, and reactions to treatment and through communication with the patient and health team members and modifies the plan as needed
o Acts as the patient's advocate, as circumstances require, by initiating action to improve health care or to change decisions or activities, which are against the interests or wishes of the patient, and by giving the patient the opportunity to make informed decisions about health care before it is provided
• Demonstrates professional accountability, as described by national lactation standards
• Demonstrates expertise in the area of lactation consultation skills
• Supervises and evaluates breastfeeding, teaching practices as demonstrated by staff to ensure consistency of teaching standards
• Initiates and receives referrals from the medical and nursing staffs, and coordinates the counseling of the breastfeeding family.

• Communicates with the medical and nursing staffs as well as other key hospital personnel the plan of care formulated with the patient to assure continuity of care

• Regular and predictable attendance is an essential function of the position.
• Triages OB clients at initial visit as needed
• Triages OB clients at time of acute complaint by phone and/or in person.
• Documents assessment and follows-up in client's Electronic Health Record.
• Responds to OB Client questions as forwarded by Call Center. / CM/ outside provider
• Reviews all OB labs as first line of triage. Adds to problem list, evaluates urgency and direct for f/u as appropriate
• Contacts clients for follow-up of abnormal labs or imaging as directed by provider.
• Assists in clinic with procedures, to include:
o Injections.
o Wound Care - packing changes, unpacking wound for MD, suture/staple removal.
o Post Void Residual, Urinary Catherization/Removal.
• Maintains logs for acute and/or ongoing management to ensure high risk clients obtain serial testing and/or imaging. Reviews infection prevention and reprocessing logs.
• Assists in clinic with immediate high-risk follow-up, referral to ED, and maintains patient compliance log. Communicates with provider after referral records obtained, arranges for follow-up, and contacts clients who are non-compliant.
• Possesses full knowledge of Obstetrical Emergency Protocols. Be willing to initiate or assist in the event of emergency.
• Demonstrates the flexibility to participate in the RN Float Pool and train at multiple sites, including triage calls and transition of care management.
General Performance:
1. Evaluates the quality of patient care through involvement in and planning of quality assurance programs.
2. Participates in yearly performance evaluation and review of job description, and seeks resources to improve professional performance.
3. Assists the Practice Leader in evaluating the performance of clinical staff.
4. Complies with MarinHealth policies and procedure for infection control, safety, administrative, and clinical practice.
Qualifications:
Education:
  • Graduate of accredited school of nursing required.
  • BSN or ADN with IBCLC required

License & Certifications:
  • Current Registered Nurse, as defined by the California Board of Registered Nursing.
  • International Board-Certified Lactation Consultant or obtain certification within 2 years of hire
  • BLS Certification (American Heart Association) required.

Experience:
  • Minimum one year of professional nurse experience in a clinic setting.
  • International Board-Certified Lactation Consultant or obtain certification within 2 years of hire
