- Provides office-based nursing care in collaboration with provider
- Assesses physical and psychosocial needs and formulates nursing plan of care
- Collects and documents current health status and assists in scheduling patient office visits
- Triages telephone calls and initiates appropriate nursing intervention
Qualifications
- Current Connecticut RN License required
- Cardio and epic experience a plus
- Previous experience in Medical/Surgical Office setting preferred
- Experience with medical software
