Job Description & Requirements Specialty: Care Manager Discipline: RN Duration: Ongoing 30 hours per week Shift: 8 hours, days Employment Type: Staff Job Summary: The RN Team Leader assumes a lead role in collaboration with a clinician partner to assist in the timely, appropriate and safe transition of patients from hospital, emergency departments, home health/hospice, and outpatient clinics to other care settings across the continuum. The RN Team Leader supports and assists with the development of a high functioning self-directed team. Practices and demonstrates skills that foster professional commitment of the team members to function to their level of licensure and training, make care decisions utilizing appropriate tools, and assumes accountability for providing safe and timely transitions of care which support Kaiser Permanentes (KPs) mission, vision and values. Essential Responsibilities: Practices clinical and leadership skills. Evaluates and prioritizes staffing needs while collaborating with manager/department assistants. Collaborates, advocates, identifies scope, workflow, policy/procedure involvement. Handles member concerns. Mentor and coach team members with issues and concerns related to patient care and workflow. Critical thinking and delegation of appropriate work. Demonstrates effective communication with patients, clinicians, interdepartmental and intradepartmental staff. Participates in the hiring, training and evaluating process. Facilitate orientation and training needs. Assists in the assessment and monitoring of clinical competencies of the team. Provides input into performance evaluations. Acts as mentor and coach in assisting team members with training or educational needs. Demonstrates personal commitment to continuous learning and professional development. Coordinates communication flow within the team and other departments within KP and plan hospitals. Participates in communicating department issues locally and regionally. Facilitates information flow when emergency situations occur. Communicates work team issues to manager on a regular basis. Communicates team performance in meeting clinical and service targets. Models and monitors for adherence to Quality of Service Standards and Behavioral Guidelines, ethical and legal behavior in all professional activities. Understands/supports and acts as a resource related to KPs policies, legal considerations and professional ethics. Adheres to and assists in monitoring teams adherence to Health Insurance Portability and Accountability Act (HIPPA), confidentiality policies and Principles of Responsibility (POR). Develop and maintain escalation paths for interdepartmental or patient/family issues. Demonstrates knowledge of and facilitates national and regional strategic initiatives at the local level. Maintain relationships with facility and external partners. Ensure staff has access to facility or systems to support job duties. Provides clinical expertise and professional nursing leadership to support the health care team in meeting the needs of the patient and safe and timely transition in care across the continuum. Understands and participates in population-based care activities which are supported by an interdisciplinary health care team. Maintains and monitors a safe and therapeutic environment for patients, clinicians and staff. Demonstrates personal proficiency in using computer-based information, Medicare criteria, , and other clinical tools to determine appropriate level of care that will meet the needs of the patient. Basic Qualifications: Experience Minimum two (2) years of skilled nursing, LTAC or acute rehabilitation experience Minimum one (1) year of medical, surgical, orthopedic, LTAC, acute rehabilitation or critical care nursing experience in an acute setting. Education BSN or bachelors degree in a health-care related field or currently enrolled in a BSN program, or four (4) years of directly related experience. High School Diploma or General Education Development (GED) required. License, Certification, Registration This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post hire. Registered Nurse License (Washington) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire Registered Nurse License (Oregon) within 6 months of hire Basic Life Support within 3 months of hire Additional Requirements: Demonstrated ability to interrelate with physicians, nurses, support staff, and patients in interdisciplinary approach. Demonstrated ability to work as part of a team and work as a constant patient advocate. Basic physical, psychosocial, functional assessment skills. Familiar with care processes and systems in settings internal and external to Kaiser Permanente. Familiar with and able to collaborate with Kaiser Permanente and community resources. Thorough knowledge of principles of teaching and delegation, physical assessment and care planning, and appropriate utilization of acute hospital, long-term care, and home care resources. Able to develop concise and thorough documentation of patient clinical assessment and care needs. Highly effective problem solving, written and verbal communication, organizational and time management skills. Familiarity with and ability to use computers. Preferred Qualifications: Minimum three (3) years of Medical/Surgical, Critical Care, or NICU Nursing. Minimum three (3) years of recent experience in one or more of the following areas: Hospital Inpatient Discharge Planning, SNF Placement, Skilled nursing, LTAC, Acute Rehabilitation, Community Health, Home Health and/or Utilization Management. Certified Case Management (CCM) is preferred upon hire/transfer. Knowledge of appropriate utilization of acute hospital and Kaiser Permanente internal resources. Knowledge of Medicare and Medicaid regulations related to eligibility requirements: hospital, nursing facilities, home health, hospice, and Durable Medical Equipment (DME). Demonstrated clinical judgment and customer-focused service skills. Knowledge of principles of patient teaching, disease prevention measures, and physical assessment as it relates to the needs of patient and the next level of care.
