Job Title: Level 2 Registered Nurse Case Manager Job Specialty: Medical/Surgical Nursing Job Duration: 13-week contract Shift: Monday to Friday, Day shift (8:00 am - 4:30 pm), weekends as needed Guaranteed Hours: Not specified Experience: Certified Home Health Care experience required License: Valid RN license Certifications: None specified Must-Have: Experience with EPIC or Netsmart Job Description: - Develop and revise treatment and discharge plans in collaboration with the patient, family, physician, care delivery team, and third-party payors, including obtaining medical orders and approval of medical services.- Complete and submit assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within Agency-defined time frames.- Process orders and notify the physician of patient needs and changes in condition. Complete certification/recertification orders and discharge summaries.- Provide an environment that promotes respect for patients, their privacy, confidentiality, and property.- Initiate appropriate preventive and rehabilitative Deliver services requiring substantial specialized nursing skills.- Consult and educate the patient, family, and other team members regarding disease processes, self-care techniques, and preventive Supervise LPN to ensure patient health, safety, and compliance with the plan of care.- Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the Home Health Agency.- Interact positively with patients, physicians, referral sources, and others to maintain ongoing Support Care Management philosophy, including collaborative customer-focused planning and case management to meet individual health and service needs and promote quality and cost-effective outcomes.- Ensure completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.- Maintain current knowledge of all available community-based services.- Demonstrate flexibility and willingness to visit patients based on need, not geography.- Ensure prompt problem resolution.- Ensure authorization and release of information form has been signed by the patient or responsible party.- Attend patient care conferences for patients assigned to your care.- Participate in achieving the Agency's operational and financial objectives.- Monitor results and progress, and take corrective action on quality indicators, including customer satisfaction with the RN function.- Understand the RN function's importance and its impact on patients, other care providers, readmission, productivity, utilization, revenue, and expense.- Pursue efforts to reduce or eliminate avoidable costs and errors.- Respond to findings of Quality Management reviews/audits and ensure resolution of potential problem areas.- Participate in peer review and Quality Assessment and Performance Improvement as assigned.- Maintain productivity requirements.- Ensure appropriate visit authorizations are in place and document the medical necessity for care in the home setting.- Contribute to an environment of professional growth, learning, trust, and mutual respect for all employees.- Promote recognition, respect, and celebration of workforce diversity.- Participate in education, including orientation, in-service training programs, and self-development as needed.- Demonstrate commitment, professional growth, and competency.- Be responsible for Key Performance Indicators (KPIs) established by management.- Perform other duties as requested.(Note: This job description is intended to provide a general overview of the position and is not an exhaustive list of all responsibilities and qualifications.)
