Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
  • Community Health Services : WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
  • Clinical Health Services : Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
  • Supportive Health Services : Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

• Reproductive Health Program Nurse—LPN or RN

Areas of Responsibilities : Work in a high volume, fast-paced environment functioning as a RN or LPN, within the scope of their license, assisting Reproductive Health patients from intake to checkout for nursing and provider visits. Ensure patients have access to confidential patient-centered care and are accurately and effectively counseled and treated. Provide excellent customer service as well as exhibit attention to detail completing clinical data entry into the electronic medical record, as well as within required agency forms and grant reports. Deliver healthcare services for the Reproductive Health Program under the guidelines of the Federal Title X grant. Coordinate patient case management in the areas of preventive screening and obstetrical services, providing client assessment, care, and education.
Primary Duties :
  • Play an active role in patient services using the provider led, team-based Patient Centered Medical Home Care Model.
  • Work as an essential member of the provider team to achieve and maintain a high level of patient care within a fast-paced high-volume environment. Ensure patients receive care in an efficient, effective, and courteous manner, supporting patient care that is "accessible, continuous, comprehensive, evidence-based, family-centered, coordinated, compassionate and culturally effective."
  • Work within the RN/LPN’s scope of practice performing skills including but not limited to; scheduling, rooming, collecting vital signs, laboratory specimen collection and processing, AlC/blood sugar/HIV/pregnancy testing, intramuscular injections, counsel patients and provide educational material, patient check out and clinical data entry in the electronic health record.
  • Act as the point person for Community Action Health Center’s prevention and chronic care case management efforts, including active communication with state-funded programs; submit monthly, quarterly, and annual reports to such programs, as required.
  • Provide OB case management to obstetrical clients including billing services to all applicable partners and follow-up.
  • Implement procedures for accurate and timely maintenance of patient's electronic health record, including but not limited to reviewing and monitoring of records for missing forms or data updates.
  • Follow all federal and state program requirements in the delivery of care; comply with reporting requirements for all applicable funders.
  • Receive and make phone calls, create and respond to phone notes and/or message(s), document patient question/issue/concern and forward to assigned provider team and prepare charts for appointments.
  • Support integrated patient care by providing both internal and community resource information and/or materials written in the patient's preferred language, for patients and their families.
  • Participate in outreach and educational events.
  • Maintain confidentiality of Community Action Health Center patient information, medical records, and communications per CAPWN policies and procedures. Follow proper guidelines when working with careful contacts.
  • Provide HIV and other STD testing, treatment, and counseling.
  • Perform screening and intake questions consisting of a highly personal nature related to reproductive health issues and STDs.
  • Provide information, conduct post-counseling documentation and follow-up in a nonjudgmental manner on contraceptives, reproductive health issues, and STDs.
  • Monitor inventory of all medications that include but are not limited to oral and injectable drugs per Nebraska Pharmacy regulations for Title X and assure patient understanding of medications prescribed along with directions for use.
  • Attend meetings, in-services and training as required.

Physical Requirements:
  • Ability to frequently bend, stoop, squat, walk and pull drawers.

  • Ability to push a wheelchair.
  • Occasionally lifting and carrying up to 30 lbs.

Minimum Qualifications :
Licensed Practical Nurse
Grade: 12
l. Current Nebraska license as an LPN.
  • Current CPR Certification.
  • Valid Nebraska driver’s license with good driving record and insured vehicle.
