Regional Goals of Care Coordinator- Social Worker or RN- REMOTE

Full-Time Regional Goals of Care Coordinator

M-F 8:30am to 5pm, REMOTE, prefer candidates living in Central or Western, PA, such as near Erie, PA or Johnstown, PA

Ideal candidate will be a Social Worker or RN and have home health and hospice experience. 

Our Social Workers,  have been called to care when they’re needed most. As a member of our home health team, you’ll support a full range of patient services to bring comfort and dignity to our clients. 

What you’ll do:  

• Provide direct patient counseling services; support crisis interventions as needed.  
• Assist patient and family in community resource planning and access.  
• Help the patient and family navigate their ongoing healthcare journey. 

What we’re looking for:  

• Education: Master of Social Work (MSW).
• Licensure: Current unrestricted license to practice as a Licensed Social Worker (LSW) in the state associated with this position required; Licensed Independent Social Worker (LISW) preferred.
• One (1) year of experience practicing as a Licensed Social Worker in a home care, hospice, or similar setting preferred.
