WHAT YOU CAN EXPECT
Coordinates reporting activities to specified government agencies relating to specific abuse or neglect of patient of any age. Supplies medical information, including completed required medical forms to community agencies for use in patient's post hospitalization and follow-up treatment. Identifies Social Determinant of Health (SDoH) needs and makes appropriate referrals to a psychosocial case manager and other disciplines that comprise the patient's treatment team; refers patients and their families to a medical case manager or nursing staff for education concerning the treatment goals and plan of care. Collaborates with a Clinical Care Manager and a multidisciplinary treatment team to individualize each patient's treatment needs in order to achieve defined goals for Collaborates with a Clinical Care Manager, physicians, and a multidisciplinary team to develop and evaluate clinical pathways-standing orders; in collaboration with a psychosocial case manager, acts as a liaison between the patient and family members with patient care issues. Works with patient's third-party insurance payer regarding non-covered services in an attempt to provide needed services in the most economical means possible. Proactively assess beneficiary psychosocial and age specific care transition needs and facilitate post-acute referrals and develop individualized psychosocial plan of care with beneficiary and family and foster team approach by working collaboratively with beneficiary, family, primary care provider (PCP) and other members of healthcare team. QUALIFICATIONS
Bachelor's Degree
OR
Master's Degree- Social Work Required 3 Years of Social Work- Required Bachelor of Social Work (BSW)
OR
Licensed Social Worker (LSW)- Required Basic Life Support Program (BLS) American Heart Association- Required
