Primary Care / Family Nurse Practitioner, Community Health Center (FQHC)
A federally qualified community health center providing comprehensive primary care, integrated behavioral health, and dental services to over 30,000 patients, which primarily include immigrants, residents of public housing, individuals experiencing homelessness, and persons living with addiction is seeking an experienced family nurse practitioner. The Nurse Practitioner supports the health centers mission to assure access to healthcare for the underserved, to engage patients in a meaningful healthcare experience through the patient-centered health home model, and to provide high quality healthcare. The Nurse Practitioner provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses, and treatment. Communicate with patients regarding testing results, follow-up care, and additional information. The Nurse Practitioner supports integrated healthcare involving behavioral health providers and dental health providers in care as appropriate. The Nurse Practitioner will achieve meaningful use of electronic health records. Requirements Minimum of three years advanced practice experience in primary care. Interpersonal skills to manage and develop the staff within the most immediate work group, and to participate on provider team. The analytical ability to review internal processes needing improvement and make suggestions and the verbal ability to communicate thoughts and actions to individual patients and to staff. Bilingual English/Spanish needed for this position.
