Inpatient position, Days/Evenings/Nights, including some weekend and holiday hours. Main Function: In cooperation with the Medical Staff, the Health System and Director of Pharmacy, participates in programs and procedures designed to promote the appropriate, safe and effective use of medications for Thompson Health patients and residents. Required Job Specific Competencies: Demonstrates skill in daily communication including effectively gathering and disseminating information. Exhibits flexibility in decision-making and prioritizing assignments. Pharmacists are knowledgeable in proactively resolving problems, making recommendations, and implementing continuous quality improvements. Mature, energetic and creative individual who demonstrates team-player qualities demanded by a rapidly changing health care environment. Possesses expertise in pharmaceutical care, organizational skills, and in managing daily workload expectations. Must show a willingness to learn, take risks, assume responsibility and pursue excellence. Possesses, maintains and enhances knowledge of all accrediting and/or regulatory agency standards (i.e. Joint Commission, DOH, etc.) Actively guards the confidentiality of sensitive information including but not limited to the patients, staff and the health system. Demonstrates the knowledge & skills necessary to provide care appropriate to the age of the patient. Possessing an in-depth knowledge of medications and their uses. Demonstrates the CARES Values and Standards of Cooperation at all times. The staff pharmacist will provide professional services and support in the following distributive and clinical areas: Medication distribution and compounding Drug Information Education and Resource Materials Formulary Management/Drug Usage Adverse Drug Reactions Medication Error Prevention Nursing/Pharmacy Liaison Clinical Computer Applications Medication Use Evaluation Program Development and Implementation Lives the CARES values at all times. Flexibility is required Qualifications: Current New York State licensure and registration required Accredited continuing education as required/defined by New York State Bachelor’s degree or Pharm. D from accredited pharmacy program required Master’s prepared preferred BCPS preferred 1-2 years of hospital, nursing home or institutional experience preferred.
