Registered Nurse - Home Health
Become a part of a caring community and make a real difference in patient's lives as a Registered Nurse in Home Health.
Responsibilities:
- Provide skilled nursing visits for home health patients, including admissions, case management, and follow-up care
- Administer ongoing care and case management, in coordination with the Clinical Manager and physician's orders
- Coordinate care with other healthcare personnel, including therapists and home health aides
- Regularly report patient care/condition/progress to physician and Clinical Manager
- Prepare and complete needed clinical documentation, including medical reports, medication order changes, and care plan updates
Requirements:
- Diploma, Associate or Bachelor Degree in Nursing
- Minimum of one year nursing experience preferred
- Current and unrestricted Registered Nurse licensure and CPR certification
- Strong organizational and communication skills
- Valid driver's license, auto insurance and reliable transportation
