Pharmacist

*Job Description:*
Eppy's IV is excited to add a full-time pharmacist to our pharmacy in Beckley, WV! As part of the infusion team, the pharmacist will provide patient-centered care tailored to each patient and their medical needs regarding IV therapy. Eppy's IV is an innovative, growing pharmacy with a state-of-the-art infusion suite servicing a broad patient base in WV and VA. The overall health and well-being of our patients matters most, which is why we are looking for a dedicated pharmacist with a strong desire to serve the community by providing excellent, compassionate care.

*Responsibilities*
1. Manage and develop IV therapy patient treatment plans - including but not limited to outpatient parenteral antibiotic therapy, TPN therapy, biologic therapy, specialty medication therapy, etc.

2. Maintains required documentation with treatment plans and clinical monitoring of IV therapy

3. Ensures compounding compliance in relation to USP 797 and USP 800

4. Collaborates with internal departments within the company, home health agencies, healthcare facilities and physicians

5. Ensures and maintains quality control and performance improvement with patient care, medication and equipment usage

6. Participates in the on-call rotation

Pay: $50.00 - $60.00 per hour
