*Responsibilities*
7 shifts on, 7 shifts off. Schedule is every other Tuesday-Monday 9pm-7am. We are seeking a Licensed Pharmacist who would enjoy working in a closed door pharmacy. Radius Living partners with long-term care facilities to offer better care for their residents. · Interprets prescription orders and obtains necessary information or clarification from prescribers and nurses
· Conducts Drug Utilization Reviews in accordance with Minnesota Board of Pharmacy Rules
· Provides direct supervision of pharmacy technicians, pharmacist-interns and any other pharmacy support personnel performing pharmacy related duties
· Performs the following duties while practicing pharmacy:
o Verifies prescription validity
o Resolves prescription discrepancies and seeks clarification when necessary
o Performs prescription verification and certification in accordance with Minnesota Board of Pharmacy Rules
· Ensures compliance with FDA and manufacturer restrictions associated with REMS drugs
· Consults with prescribers, nurses and patients about drug interactions, adverse effects, medication storage, and other requested or pertinent prescription drug information as required or necessary
· Verifies correct medication, directions for use, and other prescription information, in accordance with Radius Living Rx policy and state and federal regulations prior to dispensing
· Compounds nonsterile prescriptions in accordance with USP and state and federal regulations as prescribed by calculating, weighting, measuring and mixing ingredients
· Monitors inventory, ordering, distribution, and use of compounded medications
· Collaborates with other internal departments to resolve issues
· Meets or exceeds daily workflow expectations
· Performs other duties and responsibilities as required
*Qualifications*
Education
· Bachelor of Science in Pharmacy or Pharm. D. degree
Experience
· Prior experience in LTC pharmacy preferred
· Prior nonsterile compounding experience preferred
Skills and Knowledge
· Must have excellent verbal and written communication skills, strong listening skills, and strong interpersonal skills to work effectively with other healthcare professionals
· Must have ability to work with a high degree of accuracy and attention to detail and the ability to handle multiple tasks simultaneously and to prioritize accordingly
· Must have a desire and willingness to learn and perform new tasks
· Must be self-motivated and self-directed
· An ideal candidate will have a preference for work in a fast-paced environment where accuracy and attention to detail are critical to success
· Active Minnesota Pharmacist License
· Not on the DEA Excluded Parties List
· Not on the OIG Exclusions List
· Not on the SAM Exclusion List
· No pending felony charges or convictions for criminal offenses involving controlled substances
