Job Summary 

The Patient Navigator is a core member of the care team that also includes the patient’s medical provider, members of the primary care team, and other supportive members of the Pathways team.  The Patient Navigator is responsible for assisting with health insurance applications and assistance programs; educating patients on the services available at Northshore; assisting patients in accessing health services; ensuring barriers to obtaining care and/or social services are addressed; assisting with external referrals and/or other needs of the Medical Home.  This role will also assist patients with outreach events as needed.

Essential Functions

  • Understand the insurance programs available to patients through the Indiana Health Coverage Programs, Health Insurance Marketplace, and Medicare and the information needed to qualify.
  • Maintains use of proper ethical standards, informed consent, and rational decision-making.
  • Communicates to patients the purpose of the programs and the impact it may have on their wellbeing.
  • Accurately complete patient insurance/assistance applications and provide patient education on available services and networks.
  • Help patients problem-solve their health insurance issues and grievances.
  • Preference given to those individuals already certified as an Indiana Navigator, Certified Application Counselor and/or SHIP Counselor. Otherwise, must become certified as an Indiana Navigator and Certified Application Counselor within 3 months of employment and must complete training for SHIP Counselor within 6 months of employment.
  • Participate in outreach events as needed by providing insurance information and assistance in the community.
  • Assist with Primary Care Provider assignments or switches.
  • Assist patients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and/or social services.
  • Follow up with open referrals to see outside specialists.
  • Collaborate with patient’s entire care team to ensure positive health outcomes and that specific needs are met.
