The Community Health Center of Niagara (CHCN) is searching for an experienced LPN to join our team. The LPN delivers nursing care under the supervision of the Registered Nurse and/or Physician

Duties and Responsibilities:
General:

  • Adheres to agency policies and procedures regarding confidentiality
  • Participates in quality improvement activities
  • Communicates effectively with patients and staff
  • Maintains a professional manner in interacting with patients and staff
  • Adheres to infection control principles
  • Participates in and contributes to Quality Improvement and Quality Assurance activities and functions as needed.
Specific:
  • Maintains clean, safe, orderly work environment
  • Performs supportive duties to maintain efficient patient flow
  • Prepares patients for examinations and procedures
  • Prepares rooms for examinations and procedures
  • Assists providers with examinations and procedures
  • Performs routine diagnostic tests
  • Collects and processes specimens
  • Prepares and administers medications as directed by the health care provider
  • Reviews lab results and reports abnormal findings to the provider
  • Maintains equipment according to agency policy
  • Maintains the integrity of the medical record
  • Maintains documentation in logs as appropriate
  • Provides patient education
  • Administer immunizations for adults and children
  • Recognizes emergency situations and notifies appropriate personnel
  • Requires regular attendance
  • Attends meetings as required
  • Performs other relevant duties as assigned or requested

Physical Demands: Requires periods of sitting, standing, walking, bending and stooping. Evening work may be required.
Salary Status: Hourly; non-exempt.
Minimum Qualifications: Completion of an accredited Licensed Practical Nurse Program. Licensed as a Practical Nurse in the state of New York. Three years experience in a health care setting. Computer literacy. Proficiency in Microsoft Word, Excel and Outlook.
Preferred Qualifications: Completion of an accredited Licensed Practical Nurse Program. Licensed as a Practical Nurse in the state of New York. Five years experience in an ambulatory health care setting. Computer literacy. Proficiency in Microsoft Word, Excel and Outlook.
