Licensed Practical Nurse - Greater Grand Rapids Area


Care Plus TLC, powered by Axios HR, provides skilled, knowledgeable in-home caregivers across West Michigan. Our service expertise is providing in-home caregiving services in the comfort of your own home while helping to reduce hospitalization costs, as well as providing ongoing needs assessments and analysis to lower costs without compromising the quality of care.

We are looking to expand our medical team by adding a Licensed Practical Nurse to our Grand Rapids team. This individual will manage medicine set-ups, coordinate with our RN, and supervise caregiver visits in our patients' homes.


This person will maintain high professional standards, expectations, and processes in all settings that align with Axios HR's Mission, Vision, and Core Values.


Responsibilities of the Licensed Practical Nurse:
  • Provide direct support to the RN as requested.
  • Manage phone calls and walk-in inquiries.
  • Document nurse visits while knowledgeably and competently delivering quality nursing care.
  • Exhibit excellent customer service and a positive attitude towards patients both in the home and on the phone.
  • Work with the caregivers to effectively and efficiently schedule and plan bi-annual supervisory evaluations, based on scheduled shifts.
  • Maintains a working knowledge of applicable federal, state, and local laws and regulations and practices in a manner that reflects honest, ethical, and professional morals.
  • Performs specific LPN duties for medication visits including re-ordering, picking up, and administering medications to clients according to the medical care plan and doctors orders.
  • Utilizes observation and evaluation in assessing, training, and supervising clients and caregivers, and applies nursing judgment, consistent with practice standards, in making recommendations to the case manager and client.
  • Reports changes in the client's condition as appropriate and promptly and obtains orders for changes in the plan of treatment to respond to the client's condition.

  • Assists in care plan revisions as indicated and trains field staff caregivers when needed.
  • Participates in care conferences with other healthcare team members when needed.
  • Able to be on-call every third-week while being available to answer calls, cover shifts, and support patients and caregivers.
  • You will assist the RN with all compliance and supervisory responsibilities with the caregiving staff.
  • Must be familiar with computer skills and online charting and scheduling databases. We will provide training on these databases.

Qualifications Responsibilities of the Licensed Practical Nurse:
  • Degree from an accredited LPN program is required.
  • Must be currently licensed and registered in Michigan.
  • Must maintain an active, licensed practical occupational nurse (LPN/LVN) license in good standing throughout employment.
  • One(I)year of skilled nursing experience is required.
  • CPR certification must remain current during employment.
  • Able to work onsite Monday- Thursday, 8:00 am to 4:30
